Personally I think it's next to useless trying to have an ich free system. You are much better off just providing the fish a super healthy environment.
i have always believed there is no such thing as an ich free system... it simply doesnt make any logical sense to have a system without one specific kinda of parasite..... I would love to see a large tank thats been 'ich free' for 2 years or w.e. with several large tangs, then go and stress them out somehow, throw up a mirror, make less space in the tank etc, and then see just how 'ich free' your tank is.
Darryls right, just keep your fish healthy and stress free and their immune system will take care of itself...
QT's can still be handy though, for example sometimes fish are extremely stressed from the move or transport or w.e. and if thrown into the tank right away can just stress everything out and make matters worse. a QT will allow specific attention given to that fish until its in perfect health, then proper introduction can be made into the display... but i wouldnt put a time on QT either... couple weeks could be ok, or it might take a month or two, depends on the fish......
